(Oct. 24, 2025) – The Office of the State Fire Marshal has issued a notice about recalled fire sprinkler heads that malfunctioned at a Massachusetts assisted and independent-living facility. The faulty sprinklers failed to operate during a fire, causing 10 deaths and 30 injuries.
