New Medicare Advantage Complaint Process

Medicare Advantage 3 (Dec. 24, 2025) – The Centers for Medicare & Medicaid Services released a new online form on www.cms.gov for providers needing to report a complaint about a Medicare Advantage Plan. The online form will capture basic information about the complainant, beneficiary and Medicare Advantage plan. Provider complaints will be placed into a queue in the Complaint Tracking Module, where CMS will review and triage. The instructions can be viewed here.
