MAHA ELEVATE Funding Opportunity Anticipated Early 2026
(Dec. 24, 2025) – The Centers for Medicaid & Medicare Services intends to announce a funding opportunity in early 2026 to support the Make America Healthy Again agenda. ELEVATE, the Enhancing Lifestyle and Evaluating Value-based Approaches through Evidence model, is anticipated to provide approximately $100 million to fund three-year cooperative agreements for up to 30 proposals that promote health and prevention for Original Medicare beneficiaries. Eligible recipients include health systems, accountable care organizations, private medical practices and several other organization types.
