Kansas Department of Health and Environment Preparedness Program Survey

KDHE (April 24, 2026) – The Kansas Department of Health and Environment's Preparedness Program requests your assistance. Below is a link to the second of two surveys that inform the Kansas Jurisdictional Risk Assessment. You may complete the second survey even if you or your organization did not complete the first. According to KDHE, your input and perspective are needed because the JRA is critical for identifying and prioritizing specific threats, and you represent your community. Completing a Jurisdictional Risk Assessment is required for both the Public Health Emergency Preparedness and the Hospital Preparedness Program cooperative agreements.

The survey will take about 15 minutes to complete. The survey closes at 6 p.m. on Wednesday, April 29.
