(Jan. 9, 2025) – Our partner at the Mid-America Regional Council has notified us of a voluntary recall of the AirLife® Broselow Rainbow Tape (2025 Edition, 36-23446 Rev 3). The tape contains three major errors:
- Flumazenil is listed as 0.1 mg/kg instead of 0.01 mg/kg in the Calculation Basis "Red to Head" reference table. This is a 10-fold overdose.
- Ketamine (IV/IO for pain/analgesia) is listed at 1 mg/kg, but the appropriate pediatric analgesic dose is 0.1 mg/kg. Again, a 10-fold overdose.
- Vecuronium is listed as 0.1 mg/mL instead of the correct 0.1 mg/kg (weight-based dose) in the Calculation Basis "Red to Head" reference section.
Also note, the AirLife® Broselow Rainbow Tape (2025 Edition, 36-23446 Rev 2) was recalled in May 2025. Please take immediate action to verify the accuracy of your Broselow products and contact the manufacturer or distributor with any questions or concerns.
