(Sept. 4, 2020) – Healthcare Coalition Partners of Kansas has been contracted by the Kansas Department of Health and Environment to identify any current issues/best practices in the transition of COVID-19 positive individuals or those individuals who are in post treatment of COVID-19 from an acute care hospital to a post-acute care facility or home with a home healthcare provider.
To be successful, HCCP of KS has developed a survey to determine opinions, concerns and best practices from health care providers across the disciplines of hospitals, post-acute care facilities and local health departments within Kansas.
If you work for a hospital, public health department or post-acute care facility, please complete this survey by no later than 5 p.m. on Sept. 16, 2020.
Once the surveys have been completed, the results will be sorted and reported to all seven of the regional healthcare coalitions. Those regional coalitions will create a project team to develop a regional strategy to address the transition of individuals from an acute care facility to a post-acute facility locally or within the region.
This survey and the work that follows is intended to assist in building a local and regional collaborative effort and establishing best practices within all regions of Kansas to meet the needs of the individuals served.
