(Oct. 9, 2020) – The United Methodist Health Ministry Fund is opening a pilot project to fund four or five sites to implement community health workers as part of their care teams. The project will gather data to determine if providers can deliver a higher level of care and that families will achieve higher care outcomes when community health workers are utilized. Each selected organization will be granted up to $110,000 total over two years to hire and integrate one FTE community health worker. Kansas SHIP-eligible hospitals are encouraged to apply.
