Community Health Worker Grant Now Open
Grants (Oct. 9, 2020) – The United Methodist Health Ministry Fund is opening a pilot project to fund four or five sites to implement community health workers as part of their care teams. The project will gather data to determine if providers can deliver a higher level of care and that families will achieve higher care outcomes when community health workers are utilized. Each selected organization will be granted up to $110,000 total over two years to hire and integrate one FTE community health worker. Kansas SHIP-eligible hospitals are encouraged to apply. The Letter of Interest can be found online. Applications are due Oct. 29, 2020. Questions can be directed to Katie Schoenhoff, director of programs, at (620) 662-8586.
