Community Connections Award Overview
Hospitals do more than treat injury and illness. Every day in Kansas, hospitals are hard at work, helping improve health and access to care in the communities they serve.

The Community Connections program, a partnership between the Kansas Hospital Association and the American Hospital Association, is helping hospitals reaffirm their rightful place as a valuable and vital community resource. The Award was established by the KHA Board in 2006 to recognize and highlight Kansas hospital community programs.

Purpose of the Award
The Community Connection Award recognizes hospitals that have distinguished themselves through efforts to improve the health and well-being of their communities. This award will annually recognize six hospital-based programs (one in each region of the state) that provide a significant community benefit.

Criteria
The award will be given to KHA member hospitals for their Community Connections programs that address the following areas: Leadership; Community Need; Community Benefit; Creativity and Impact.

The Process
Hospital stories received from KHA members are considered by the KHA District Chairs and Vice Chairs. Their recommendations are then presented to the KHA Executive Committee for approval. One award will be granted in each hospital district, for a total of six awards annually. There is no limit on the number of Community Connection programs hospitals can enter.

Award Presentation and Recognition
In recognition of their efforts to improve the health and well-being of their communities, each hospital receiving a Community Connection Award will be honored by:
     • Recognition at a KHA-hosted media/award recognition program in their community;
    • $1,000 contribution to their community program;
    • Community Connection Award;
    • Awardees highlighted during KHA Spring District Meetings;
    • Statewide Media Announcements;
    • Coverage in KHA publications and on the KHA Web site.

Materials to be Submitted
Hospital story entries should address the questions on the entry form in 500 words or less. Hospital Community Connections entries may be e-mailed to csamuelson@kha-net.org, faxed to (785) 233-6955 or mailed to:

Deadline
The completed hospital story/award entry form must be postmarked no later than March 1.
